Are you new to Therapy Place and want to register to see a Therapist? Follow the steps below to register as a new client.

This DOES NOT guarantee that you will become a client or that an appointment will be scheduled with you.

Step Two

Fill out all the required information in RED and click Register button.

After successfully entering your required information and clicking Register, you will see this screen and you will be sent an email from ClinicTracker Patient Portal titled ClinicTracker Portal Login Activation confirming your registration.

STEP 3 – CHECK YOUR EMAIL / SPAM MAIL

Check your email and confirm your registration by clicking the link in the email.

STEP 4

After clicking the email link you will be taken to the Patient Portal Registration Form. Please fill out all of the required information on the Demographics tab and the required Insurance information on either one or all three Insurance Tabs and click Save at the bottom of the form.

  • Accessing Required Forms and Paperwork
  • View Your Payment History
  • Pay Co-Pays and View Statements Online

  • Access to Educational Resources from your Therapist
